After completing the form, send it to the CDA office by fax on (02) 9890 1912 or you may also post the form to our Northmead office. Someone from TAD will contact you either by phone or in writing as soon as possible after the application is received.

In Sydney, TAD's Custom Designed Aids team can, if deemed necessary, come to the client’s home or workplace to assess the nature of the problem, by appointment. There is no extra charge for this service.

If you are in a country area serviced by a local TAD Branch or Interest Group, contact the Sydney office for a referral.
